City Health Partnership is staffed by dedicated physicians,
nurses and other local area healthcare professionals, who volunteer
their time to provide quality care to the community. Our services
are for low-income residents of central Kane County who do not have
Medicare, Medicaid/Public Aid or who are underinsured. You can find us in downtown St. Charles near the intersection of Route 31 (Geneva Road) and Route 64 (North Avenue). If you would like to help your community by making a donation or volunteering your time, please contact us by phone or email. |

City Health Partnership does not provide the following services: • Well baby exams and immunizations • OB/GYN Care • Mental Health Care • Physicals of any kind • Prescribing or dispensing narcotic, psychotropic agents or controlled substances |

of Operation Monday through Thursday 9:00 a.m. to 4:00 p.m. Limited evening hours are available. Patients seen by appointment only. |
